Outreach & Befriending Service

• For older adult who have Dementia or similar care needs we Serve SW Birmingham & bordering Bromsgrove district, providing the following benefits:

• Very caring, fully trained, supervised and DBS checked staff.

• Individual service users and primary carer’s are involved in care planning to meet their needs and wishes.

• Outreach staff work together with family, health and social care workers to achieve the best possible outcome for each person.

• Flexible day time calls / visiting on week days.

• Two course lunch and refreshments throughout the day.

• Escorted door to door transport in partnership with Birmingham Ring & Ride.
